Troop Activity Requests
"Change a Life, It's Forever"                                                  

Troop Activity Requests should be filled every time the troop has an outing that requires them to  leave their regular meeting place. The only exceptions are Service Unit and Citrus Council events. Please remember that a parental permission slip must be filled out for every event.

Troop Activity Requests are signed by the following Team members:

  • Camping/Sleepovers - The Outdoor Consultant

  • Out-of-Town Trips - The Service Unit Manager

  • All other troop activities - Your School Coordinator

The Description of activity is where you are going and what you plan on doing i.e. tour, educational class, etc.

Times should include the whole time period that the girls are in your care not just the length of the tour/activity.

You need to list all the drivers and/or backup drivers .
 
All adults attending should be listed including girls who have graduated high school.

In addition to the Troop Activity Request leaders must submit the following forms for these activities: Forms are available on the Forms Section of the website.

  • Swimming in the Ocean - Ocean Swimming - Guidelines and Request for Permission
  • Going to a Water Park - Commercial Water Attractions Guidelines and Request for Permission
  • Horseback Riding - Release and Hold Harmless Form
  • Picking up Litter for Community Service - Litter Cleanup Projects Request Form

ALWAYS include an Emergency Contact Person. It should be someone who is NOT on the trip with you.

A First Aider is only necessary if the outing that you are going on requires the need. Trips to places like Publix, the fire station, to plays etc. don't need a First Aider. Any outside outing or an inside one that has a lot of active movement should have a First Aider accompany the troop.

All sleepovers, lock ins, or campouts require someone accompanying the troop who has had the Outdoor Education Trainings. This does not have to be the leader.

It is highly recommended that the First Aider and the Camping Adult not be the same person.

Remember to have a girl member of the troop sign the form. It is nice to go over the form with  the girls so that they can learn about how things work and have each one take turns signing a form.

Once the request is signed, the troop leader keeps the pink copy of the form and carries it with her on the outing. The other two copies are kept by the school coordinator or outdoor consultant.
