Request Form
ALWAYS include
an Emergency Contact Person.
It should be someone
who is NOT on the trip with you.
A First Aider
is only necessary if the outing that you are going
on requires the need. Trips to places like Publix,
the fire station, to plays etc. don't need a First
Aider. Any outside outing or an inside one that has
a lot of active movement should have a First Aider
accompany the troop.
All sleepovers, lock
ins, or campouts require someone accompanying the
troop who has had the Outdoor Education
Trainings. This does not have to be the leader.
It is highly
recommended that the First Aider and the Camping
Adult not be the same person.
Remember to
have a girl member of the troop sign the form. It is
nice to go over the form with the girls so
that they can learn about how things work and have
each one take turns signing a form.

Once the request is
signed, the troop leader keeps the pink
copy of the form and carries it with her
on the outing. The other two copies are
kept by the school coordinator or
outdoor consultant. |
